Memberships
Direct Primary Care is not health insurance and is not a substitute for comprehensive health insurance coverage. A Nomad Health membership covers the primary care services described below only — it does not cover hospitalization, emergency services, specialist care, or major medical needs. We encourage every member to maintain health insurance (for example, a high-deductible or catastrophic plan) alongside membership.

¶Care is triage-first: whether a call leads to an in-clinic visit (by appointment), labs, a telehealth visit with a prescription, or an in-home dispatch is decided by the provider at their clinical discretion. Scheduling, same-day, and telehealth response are subject to provider schedule availability.
‖On Essential, in-clinic visits beyond the 2 included each month are $50 per visit. Nomad Home members pay a fixed discounted member rate of $175 per field-clinician visit beyond their included monthly visit.

Our Staff
Nora B. Khoury, MD
Dr. Khoury is a board-certified internal medicine physician (American Board of Internal Medicine) and expert endocrinology specialist who served as Chief Fellow of her endocrinology fellowship and Chief Resident of her internal medicine residency. Her career spans international medicine — from emergency and surgical posts in Ireland's Royal College of Surgeons teaching-hospital network to directing fleet-wide medical operations for a major cruise line — alongside urgent care, occupational medicine, and telemedicine practice across Florida. She is an active clinical researcher, serving as Principal Investigator on internal medicine and endocrinology studies, with peer-reviewed publications and international conference presentations. Her clinical focus includes diabetes and CGM management, thyroid dysfunction, hormonal and adrenal disorders, women's health, and weight management.
Steven A. Pino, PMD
Steven brings years of experience as an international critical care flight paramedic, managing complex patient care in demanding environments around the world. He spent many years as a leader in the 911 response system, including experience leading incident command during critical incidents, and he has practiced in both urgent care and in-hospital settings. Throughout his career, Steven has been dedicated to connecting vulnerable patients — including those facing addiction and homelessness — with the resources they need. As Director of Clinical Operations, he directs the practice's clinical operations so every visit — at home or over video — runs safely and smoothly, coordinating care and resources behind the scenes and advocating for every member he serves.
Trevor Portiner, PA
Trevor has spent years in urgent care and concierge medicine, delivering exceptional care to high-profile clients who expect nothing less than a white-glove standard. That experience spans the full breadth of what urgent care demands — from acute, in-the-moment concerns to the everyday health issues members bring in between routine visits. It has also shaped his approach to medicine: attentive, discreet, and built on real relationships rather than rushed appointments. For Nomad Health members, that means same-day evaluations when something comes up, thorough visits that are never rushed, and the continuity of seeing a clinician who already knows their history.
